Shared Care Close to Home—OB Pathway

Through our Shared Care OB Pathway, most of the care through your first two trimesters occurs here at home. Our expert team will share resources to help you select a delivering physician at:

Stormont Vail Health, Topeka
University of Kansas Hospital, St. Francis Campus, Topeka
Mosaic Life Care, St. Joseph
Or a facility of your choice

During your initial visit, we will review a detailed plan for all three trimesters of your pregnancy, including clinic visits, lab tests, ultrasounds and more that can be done, here in Sabetha.

First Trimester (weeks 1-12)
Goal: Confirm pregnancy, establish care, initiate labs and early imaging.

Second Trimester (weeks 13-27)
Goal: Monitor pregnancy progression and choose a delivering facility.

Third Trimester (weeks 28-40)
Goal: Increase monitoring, prepare for delivery, and fully transition care to the delivering provider.

Postpartum
Once your baby arrives, you’ll return to Sabetha Family Practice for postpartum care and your child’s ongoing well-child checks.




Breastfeeding Support Services

We are proud to partner with Nemaha County Community Health to offer comprehensive breastfeeding support for families right here in our community. The journey to a successful breastfeeding experience starts before your little one even arrives! By scheduling an appointment as you near the end of your pregnancy, you can build a relationship with a team dedicated to your success. From prenatal preparation to post-delivery guidance, we are here to support you and your newborn every step of the way.
