Social ServicesThe Social Services department at the Sabetha Community Hospital helps patients and their families cope with the difficulties associated with disease and illness. Our job starts the day you are admitted into the hospital when we begin to evaluate what services or assistance may be needed upon discharge. Services offered include:
Patient AssessmentSocial Workers assist patients and families in identifying ways to cope with all psychosocial aspects of illness or injury.
Admission PlanningSocial Workers address problems that delay or interfere with admissions to Sabetha Community Hospital.
- Medicare, Medicare Drug Plan, Medicaid and other insurance issues
Advanced DirectivesSocial Workers support a patient's right to participate in making decisions about their healthcare. They provide patients and families with information about advance healthcare instructions, including Living Wills, Healthcare Treatment Directives and Durable Power of Attorney. Discharge PlanningSocial Workers assist in developing and implementing individual plans to assure your care continues after you leave the hospital. Our Social Services department can help you arrange:

- Rehabilitation placement
- Home Health & Hospice care
- In-home community services
- Meals on Wheels
- Legal issues, such as guardianships and abuse and neglect (child and elderly)
- Support Groups
- Area Agency on Aging
- Veterans benefits
- Medicare, Medicaid, Medicare Part D and Social Security Benefits
- Prescription Drug Programs
